Ohio-based trucker shut down following fatal crash while under the influence
FMCSA says Gregory Alan Barnhart was driving on State Route 534 in Milton Township, Ohio, on Sept. 28 when his truck crossed the center line into oncoming traffic and hit a cargo van, killing its driver and a passenger.

Two-truck Alabama fleet shut down following fatal crash, violations
Jeffery Finklea of Beatrice, Ala., was ordered to shut down his two-truck operation on April 24. FMCSA says a truck operated by J&L crossed the center line of Alabama Highway 13 on March 28 and collided head-on with a passenger car, killing both of its occupants.

Kentucky trucker shut down after causing accident, leading police on chase
Dalton McCoy Lampley was driving eastbound on I-24 in Kentucky on March 17 when he crossed the median into the westbound lanes. After striking two cars, Lampley continued driving with police in pursuit, leaving the interstate and driving into Calvert City, Ky., according to FMCSA.
