California driver shut down after failed drug tests, crash
The driver was found to be medically unqualified to operate a commercial vehicle during a May 2015 FMCSA investigation in which the agency discovered he’d tested positive for the use of prohibited controlled substances, amphetamines or methamphetamines, on three separate occasions since 2012.

Carrier asks FMCSA for exemption from short hauler log-keeping rules, blames detention time
The request would exempt Farruggio’s drivers who stay within the 100 air-mile radius but occasionally exceed the 12-hour limitation from having to complete a daily record of duty status. The company says approximately 12 percent of its drivers exceed the 12-hour limitation on a weekly basis, primarily due to waiting times at rail yards and piers.

NTSB investigators blame significant hours violations, false logs and scant FMCSA oversight for deadly 2014 crash
NTSB investigators presented their findings to the full Board, determining that trucker fatigue and logbook falsification contributed to the crash, as well as “inadequacy of Federal Motor Carrier Safety Administration efforts to address the safety deficiencies of high-risk carriers or prioritize action to halt their operations.”
